Work Permit Information
ANY STUDENT UNDER THE AGE OF 18 THAT IS WORKING AT ANY TIME MUST HAVE A WORK PERMIT
Blank Work Permits are available at EKHS in the CTE area of the Guidance Office or at Door A.
Work Permit Instructions:
· If you are UNDER 16, you need a CA-6 work permit for minors UNDER 16 years of age. Must be printed back to back (instructions on the back) in Landscape Format.
· If you are 16 or 17, you need a CA-7 work permit for minors 16 and 17 years of age. Must be printed back to back (instructions on the back) in Portrait Format.
· If you are 18, you DO NOT need a work permit.
Instructions for filling out your work permit:
Section I: This section must be completed by the student.
Section II: This section must be completed by the employer.
*Section III: This section will be completed by Kentwood Public Schools AFTER you and your employer complete Section I & II.
*Once Sections l and ll are completed, you need to bring the work permit to EKHS in the CTE area of the Guidance Office. If it is completed correctly, the work permit signing officer will process your permit in a timely manner. Once it is processed, you will need to pick it up and return it to your employer.
Make sure every blank space is filled out correctly in BLUE or BLACK INK.
Please be advised that it may take up to 48 hours to process your work permit. Please plan accordingly. You may not start working until your work permit is completed and returned to your employer.
