Tax Form 1095-C
Tax Form 1095-C is available for all covered individuals and full-time employees via request to the Kentwood Public Schools Insurance Office. You may submit your request via telephone at 616-455-4400 x. 79913, email to kps.insurance@kentwoodps.org, or via mail.
